| NHS PHYSICAL EXAM DATES AND FORMS... |
| All of the forms for Athletes will be available after May 1st at the intermediate and MS offices, as well as at the HS in the Athletic Department office, in the Athletic Training rooms, and in the hub. The forms may also be printed off of the Student Athlete Resources page. These forms must be completed on both sides and signed by the Student Athlete and Parent or Guardian, prior to any participation, including preseason conditioning, weight training or practice.
All Student Athletes will be
required to use the forms that we provide for physical
exams and emergency information / PHI. Student
Athletes who are planning to participate in the physical
screenings at the Evan’s Complex are required
to have the history side of the physical evaluation
form completely filled out and signed by both the Student
Athletes and the Parent Guardian prior to arrival.
It is also required that they turn in their completed
emergency information / PHI form at this time.
No exam can be done unless both of these forms are completed
and signed prior to the time of the exams. The
exams will be available at Evans Athletic Complex at
the cost of $10. Please make checks payable to
NHS Athletic Training Department! Exam Dates are
as follows:
